We can help with car hire in Madagascar but only in the area of Antananarivo either the airport or city (downtown). Car hire in Madagascar is not the cheapest but you should be able to find a car that suits you and the local driving conditions.
You will need an international driving permit to be able to drive in Madagascar.
Do note that a lot of car hire companies in Madagascar only allow chauffeured hire and the chauffeur will be an additional cost. It's not always immediately obvious from the "front page" of the website so check the terms & conditions of hire carefully. Budget only allow chauffeured hire.

Most cars will come with third party insurance and collision damage waivers. Excesses vary but can be high, up to around € 2,500 for 4x4s and minivans. The usual areas excluded from damage waivers - tyres, windscreen and underside will be at greater than normal risk driving in Madagascar.
Consider car hire excess insurance to cover these areas. If you opt to employ a chauffeur, it would be worth checking whether you would still be responsible for damage to the car when it is not under your control. Some car hire excess insurance policies will cover any driver named on the car hire agreement and you don't have to specify names, but this means you will need to make sure your chauffeur is named as a driver on the rental agreement.
Do note that most car hire excess insurance will not cover you for damage when driving on unsealed roads.
